(help voter select reason) ===> Verify all required fields have been entered. ------------------------------------------------------------------------ Tell voter to complete the top section of the provisional envelope. ===> Verify all required fields have been entered. ------------------------------------------------------------------------ Complete the bottom of the provisional envelope: * Correct precinct number for this voter (per street book) * Issued ballot style (always 7) * Your signature at the bottom left ------------------------------------------------------------------------ Complete provisional poll book page: * Voter's name, address, and issued ballot style (always 7) ------------------------------------------------------------------------ Place PIN labels: * On provisional poll book line * On provisional envelope * On "Voter Call-in Instruction" form (don't give to voter yet) ------------------------------------------------------------------------ Complete provisional envelope * Write provisional poll book number on bottom section (left) * Write precinct number (always (01-43) on bottom section (right) ------------------------------------------------------------------------ Write voter's correct precinct number on ballot * This need NOT be 01-43 ------------------------------------------------------------------------ Tell voter to how to handle the ballot: * Give him ballot (prefolded, unless will use AutoMark) and envelope * Tell him to mark the ballot appropriately (fill out the ovals) * Tell him to then seal the marked ballot inside the envelope * Tell him to bring the sealed envelope back to the Help Table * Emphasize the envelope with ballot inside must be returned here, not put in the M100 * Direct him to the nearby provisional voting booth or table to mark the ballot ------------------------------------------------------------------------ Finish up with this voter after he has returned the sealed envelope * Paperclip any non-ballot supporting material to the sealed envelope * Put the sealed envelope (and paper-clipped material) in the provisional ballot case * Give the voter the "Provisional Voter Call-in Instructions" form containing his PIN label ======================================================================== In addition to this we had a marked-up provisional envelope with the required voter fields highlighted in one color and the required precinct official fields in another color.
