Voting a Provisional Ballot (Fall 2012)
(just renumbered items to match numbering in Fall 2012 manual)
- 1 Give voter Provisional Ballot Envelope
- Instruct voter to fill in fields highlighted in yellow
(white fields are optional)
- Instruct voter NOT to detach top section from envelope
- 1 Ask voter to complete and sign top section of envelope
(Registration/Update form)
- Point out the often-overlooked citizenship and age section

- 1 Help voter select exception reason in bottom section of envelope
- 1 Ask voter to complete and sign bottom section of envelope
- 1 For primary election, if voter is unaffiliated, ask voter for party selection
- 1 Verify voter has completed all required fields, including signatures, in both sections
- 2 Use laptop Street Search to look up voter's residence address from top section of envelope
- 2 Write precinct number of residence address on bottom section of envelope

- 10 Double check that you and voter have filled in all required (color-highlighted) fields
- 10 Fold ballot, insert it in envelope, and hand envelope to voter
- 10 Instruct voter:
- Use Help Table voting booth
- Mark ballot with BIC pen -- fill ovals
- Seal ballot in provisional envelope
- Return envelope to Help Table
- Do NOT put ballot in scanner
- 11 When voter returns with envelope, give him/her Provisional Voter Instructions form
- 12 Give voter an
