Voting a Provisional Ballot  (Spring 2012)

• 1  Give voter Provisional Ballot Envelope
  • Instruct voter to fill in fields highlighted in yellow  (white fields are optional)
  • Instruct voter NOT to detach top section from envelope
• 1  Ask voter to complete and sign top section of envelope (Registration/Update form)
  • Point out the often-overlooked citizenship and age section upper left corner of top section
• 1  Help voter select exception reason in bottom section of envelope
• 1  Ask voter to complete and sign bottom section of envelope
• 1  For primary election, if voter is unaffiliated, ask voter for party selection
• 1  Verify voter has completed all required fields, including signatures, in both sections

• 2  Use laptop Street Search to look up voter's residence address from top section of envelope
• 2  Write precinct number of residence address on bottom section of envelope bottom left

•  

• 3  Write voter's name and address on Provisional Poll Book entry
• 3  Sequentially number the Provisional Poll Book entry
• 4  Place 1. POLL BOOK label on Provisional Poll Book entry for voter right side -- PIN Label
• 4  Place 2. ENVELOPE label on provisional envelope top right of bottom section -- PIN Label
• 4  Place 3. INSTRUCTION SHEET label on Provisional Voter Instructions form
• 5  Write Provisional Poll Book Number for voter on envelope top left of bottom section
• 6  Write your precinct number in Precinct Where Voting on envelope top right of bottom section

• 7  Get appropriate ballot from Ballot Table using Street Search results from Step 2
  • For ballot selection, consider voter's age and (for primary election) voter's party affiliation
• 8  Write ballot style actually issued on Provisional Poll Book page
• 8  Write ballot style actually issued on bottom section of envelope bottom middle
• 8  Sign bottom section of envelope bottom right

• 9  Write precinct number (from Step 2) of voter's residence address in ballot PCT VTD area top
  • Write nothing else on the ballot
• 9  Double check that you and voter have filled in all required (color-highlighted) fields
• 9  Fold ballot, insert it in envelope, and hand envelope to voter
• 9  Instruct voter:
  • Use Help Table voting booth
  • Mark ballot with BIC pen -- fill ovals
  • Seal ballot in provisional envelope
  • Return envelope to Help Table
  • Do NOT put ballot in scanner

•  

• 10  When voter returns with envelope, give him/her Provisional Voter Instructions form
• 11  Give voter an I Voted sticker
• 12  Provide further details about why voter is voting provisionally large blue section in top right
• 12  Paper-clip any supporting material, e.g., ATV, to outside of voter's sealed envelope
• 12  Place sealed envelope (with any attachments) in Provisional Ballot red plastic bag
